Is ultrasound really the higher the dimension, the better?

3D technology has been deeply rooted in people's minds today. Watching movies depends on 3D, and painting exhibitions are also 3D, and 3D printing technology. With the rapid development of medical technology, ultrasound examination has also developed from traditional A-mode ultrasound (one-dimensional ultrasound) and B-mode ultrasound (two-dimensional ultrasound) to three-dimensional ultrasound and four-dimensional ultrasound. Every patient who comes to the hospital not only wants to enjoy a warm medical environment, but also expects doctors to provide the most sophisticated medical technology. In the Department of Ultrasound, I often encounter pregnant mothers and ask: "Doctor, are we doing three-dimensional ultrasound? Can you see anything? Can you take a picture of your baby's face and show me how good it is? ”

For these problems raised by pregnant women, the ultrasound doctors are sometimes very helpless. The examination time is limited, and there are many people waiting in line. One sentence and two sentences cannot explain clearly. Today, the doctor of Xie Meng, affiliated to the Obstetrics and Gynecology Hospital of Fudan University, will give you a specific explanation. From one-dimensional to four-dimensional, is ultrasound really the higher the dimension, the better?

1. One sentence tells you the difference between one-dimensional, two-dimensional, three-dimensional, four-dimensional ultrasound

Ultrasound began with A-mode ultrasound, which is a one-dimensional ultrasound. It was popularized in China in the early 1960s. It only shows the echo amplitude of different tissues, and there is no visual imaging of organs.

In the 1970s, the appearance of B-mode ultrasound, or two-dimensional ultrasound, replaced A-mode ultrasound. So far, two-dimensional ultrasound is still the main method of ultrasound diagnosis.

The three-dimensional ultrasound technology that everyone pays attention to is imaging on the basis of two-dimensional ultrasound, which must be combined with two-dimensional ultrasound, or reconstructed by computer with two-dimensional ultrasound images. Therefore, the quality of the two-dimensional image directly affects the effect of three-dimensional imaging.

Similarly, four-dimensional ultrasound adds a time axis based on three-dimensional ultrasound, which is dynamic three-dimensional imaging, and therefore is inseparable from two-dimensional ultrasound.

2. Does fetal large malformation ultrasound require three-dimensional or four-dimensional ultrasound?

Pregnant mothers must first understand that the purpose of prenatal ultrasound is to find out if the fetus has structural abnormalities, not to see the baby's long hair.

In addition to the requirements for ultrasound equipment, the correct rate of prenatal diagnosis is affected by various factors such as doctor's level, gestational age, amniotic fluid volume, abdominal wall thickness of pregnant women, type of fetal malformation, fetal position, and time spent on examination. Not everyone understands: 3D, 4D is more clear than 2D.

Three-dimensional ultrasound is only an auxiliary examination method when two-dimensional ultrasound examination has abnormal findings. For example, when two-dimensional ultrasound finds fetal cleft lip, three-dimensional ultrasound imaging can be used to obtain three-dimensional images of the fetal lip, so that pregnant women and their families are more intuitive. Understand. However, the three-dimensional ultrasound is used for fetal examination. The first condition is that the two-dimensional image is clearly displayed and the fetal position is highly matched. If the fetal position is not good or there are more fetal movements when doing three-dimensional imaging, some artifacts will appear in the three-dimensional imaging.

3. When can 3D and 4D ultrasound be used?

The current three-dimensional and four-dimensional ultrasonography includes the diagnosis of uterine malformation, the detection of ovarian volume and follicles in polycystic ovary syndrome, high-risk ultrasound examination of the fetus, and fetal heart ultrasound. These tests are used as an auxiliary diagnostic for conventional two-dimensional ultrasound and do not replace two-dimensional ultrasound. All patients or pregnant women must first perform three-dimensional or four-dimensional imaging based on the diagnosis of two-dimensional ultrasound.

4. The US Food and Drug Administration (FDA) issued a warning for commercial fetal ultrasound photography

As early as 2000, the International Society of Obstetrics and Gynecology Ultrasound (ISUOG) issued a safety statement for "Fetal Ultrasound"; required prenatal ultrasound to follow the ALARA principle (As low As Reasonably Achievable); 2014 US Food and Drug Administration (FDA) A warning is again issued for commercial fetal ultrasound photography. Ultrasound is safe for the fetus, but avoid unnecessary ultrasound.
